The parameter is always zero except when registering the three internal
io regions (ROM, unassigned, notdirty). Remove the parameter to reduce
the API's power, thus facilitating future change.

The ARMv7-M NVIC device pokes itself into the CPU state. Now we have a
proper device model we can have the CPU/SoC code do this.

We have both IRQ sinks and GPIO inputs. These are in principle exactly
the same thing, so remove the former.
