Add p_set_proto_data, which either updates our entry if we have a
proto+key match or adds an entry if we don't. Use it with
p_set_proto_depth. Document it and our other proto_data routines.
If a header declares a function, or anything else requiring the extern
"C" decoration, have it wrap the declaration itself; don't rely on the
header itself being included inside extern "C".
Add convenience routines for getting and setting a per-protocol,
per-packet depth value, which can be used to limit recursion, nesting,
cycling, etc. Use them in the BACapp, DAAP, Mongo, VLAN, and WBXML
dissectors.

Those are the only ones meaningful. Let's convert the buggy dissectors
and add an assert to avoid the misuse of the pool parameter in the future

It's not tied to the frame_data structure any more, so it belongs by
itself.
Clean up some #includes while we're at it; in particular, frame_data.h
doesn't use anything related to tvbuffs, so don't have it gratuitiously
include tvbuff.h.
