kvm: x86: Refactor use of interrupt_bitmap

Drop interrupt_bitmap from the cpustate and solely rely on the integer
interupt_injected. This prepares us for the new injected-interrupt
interface, which will deprecate the bitmap, while preserving
compatibility.
