They're not just addresses, they also include a mask length for IPv4 and
a prefix length for IPv6. Rename them appropriately.
Rename the old ipv4_addr_and_mask() and ipv6_addr_and_mask() to reflect
that 1) they fetch data from a tvbuff and 2) *don't* fetch the mask
length or prefix length, those lengths are passed as arguments to
indicate how many bytes worth of address to fetch.

From me: Fix a number of instances where the function prototype or
the function definition wasn't changed so there was a mismatch
thus causing Windows (but not gcc) compilation errors.

Mikus. Add a buf_len parameter to ip_to_str_buf(), and make sure it's
enforced. Copy the release notes over from the 0.99.5 trunk and add a
note about the ISUP dissector (which is affected by the overrun).

overrun that Coverity thought existed does not exist. But I did
notice something else: a signed/unsigned disparity between
net_bits and new_nmask_bits.
So I fixed that, then realized that the function in question, a static
function named created_mask, is only used by one other function in
the file. So I got ride of create_nmask and moved its logic into the
single calling function, ipv4_addr_set_netmask_bits.

they have LF at the end of the line on UN*X and CR/LF on Windows;
hopefully this means that if a CR/LF version is checked in on Windows,
the CRs will be stripped so that they show up only when checked out on
Windows, not on UN*X.
