These new etype values (19 and 20) are described in RFC 8009.

- when building the END hash entry, always use the BEGIN destination
address. If no match is done, we will search using the BEGIN entry anyway
- add the source PC to the END hash entry. As it will be used for
messages initiated by the host that sent the BEGIN, we probably can
assume that the PC will not change

Rename range_foreach_r() to range_foreach(), getting rid of the old
range_foreach().
If your callback doesn't require an additional argument, just pass NULL
when calling range_foreach(), and declare the argument as unused.

"enterprise-numbers" is converted to tab-separated values and renamed
"enterprises". Unused fields are stripped.
PENs are stored in a hash table loaded at run-time.
User "enterprises" file is loaded from the personal config dir.
Misc make-sminmpec.pl improvements and fixes.
Note: names of type "Entity (formerly ...)" have the formerly part commented out for a cleaner output.

The 'U-RNTI' field in RLC Info struct is both used in the code and shown in the UI as a generic unique 'UE ID' (not specificly U-RNTI, although sometimes it is)
This commit renames the field to fit it's usage.

Those algorithms are e.g. used to sign X.509 certificates, or in CMP (RFC4210)
messages.
It is not fully deterministic to which dissector those OIDs should be added,
PKCS1 is chosen as there are already the OIDs for the ECC curves and the OIDs
in question are also listed in PKIXAlgs-2009.asn.

When reviewing the code, the following issues were identified:
- otid/dtid on 3 bytes were not stored
- when receiving the first continue from dest, the TC_END hash entry was
created with the source tid / address instead of destination ones
- when receiving the first continue from src, the logic could prevent
the creation of the hash entry

This was inspired by using the Decode-As UI to decode Field "SSL TCP Dissector"
Value (port) XXX as YYY. "SSL Port" makes more sense as the UI name.
