The arch_ioremap function on i.MX is now an indirect function pointer.
In order to use it from any loadable module, the pointer itself
has to be exported.

53cbcbcf43 ARM: 7166/1: Use PMD_SHIFT instead of PGDIR_SHIFT in dma-consistent.c
Commit 99d1717d (ARM: Add init_consistent_dma_size()) introduces dynamic
allocation of the consistent_pte array. The number of PTEs should be
calculated based on the number of PMD entries rather than PGD, hence the
PMD_SHIFT.

2674dd0b1c ARM: 7163/2: PL330: Only register usable channels
When the manager is running non-secure, the only channels that can
issue interrupts are the ones that have a 1 in their corresponding bit
in Configuration Register 3. The other ones will generate an abort
when trying to signal the end of the transaction so they are useless
in non-secure mode.

fa0ce4035d ARM: 7162/1: errata: tidy up Kconfig options for PL310 errata workarounds
The Kconfig options for the PL310 errata workarounds do not use a
consistent naming scheme for either the config option or the bool
description.

This patch tidies up the options by ensuring that the bool descriptions
are prefixed with "PL310 errata:" and the config options are prefixed
with PL310_ERRATA_, making it much clearer in menuconfig as to what the
workarounds are for.

11ed0ba175 ARM: 7161/1: errata: no automatic store buffer drain
This patch implements a workaround for PL310 erratum 769419. On
revisions of the PL310 prior to r3p2, the Store Buffer does not
automatically drain. This can cause normal, non-cacheable writes to be
retained when the memory system is idle, leading to suboptimal I/O
performance for drivers using coherent DMA.

This patch adds an optional wmb() call to the cpu_idle loop. On systems
with an outer cache, this causes an explicit flush of the store buffer.

95ef1e5292 KVM guest: prevent tracing recursion with kvmclock
Prevent tracing of preempt_disable() in get_cpu_var() in
kvm_clock_read(). When CONFIG_DEBUG_PREEMPT is enabled,
preempt_disable/enable() are traced and this causes the function_graph
tracer to go into an infinite recursion. By open coding the
preempt_disable() around the get_cpu_var(), we can use the notrace
version which prevents preempt_disable/enable() from being traced and
prevents the recursion.

Based on a similar patch for Xen from Jeremy Fitzhardinge.

bce34d1442 ARM: perf: initialise used_mask for fake PMU during validation
When validating an event group, we call pmu->get_event_idx for each
group member in order to check that the group can be scheduled as a
unit on an empty PMU.

As a result of 3fc2c830 ("ARM: perf: remove event limit from
pmu_hw_events"), the used_mask member of struct cpu_hw_events must be
setup explicitly, something which we don't do for the fake cpu_hw_events
used for validation.

This patch sets up an empty used_mask for the fake validation
cpu_hw_events, preventing NULL deferences when trying to get the event
index.

8bf00a5299 KVM: VMX: add support for switching of PERF_GLOBAL_CTRL
Some cpus have special support for switching PERF_GLOBAL_CTRL msr.
Add logic to detect if such support exists and works properly and extend
msr switching code to use it if available. Also extend number of generic
msr switching entries to 8.

KVM on s390 always had a sync mmu. Any mapping change in userspace
mapping was always reflected immediately in the guest mapping.
- In older code the guest mapping was just an offset
- In newer code the last level page table is shared

1eddb85f88 KVM: s390: Fix tprot locking
There is a potential host deadlock in the tprot intercept handling.
We must not hold the mmap semaphore while resolving the guest
address. If userspace is remapping, then the memory detection in
the guest is broken anyway so we can safely separate the
address translation from walking the vmas.

9e6dabeffd KVM: s390: Fix RUNNING flag misinterpretation
CPUSTAT_RUNNING was implemented signifying that a vcpu is not stopped.
This is not, however, what the architecture says: RUNNING should be
set when the host is acting on the behalf of the guest operating
system.

CPUSTAT_RUNNING has been changed to be set in kvm_arch_vcpu_load()
and to be unset in kvm_arch_vcpu_put().

For signifying stopped state of a vcpu, a host-controlled bit has
been used and is set/unset basically on the reverse as the old
CPUSTAT_RUNNING bit (including pushing it down into stop handling
proper in handle_stop()).

a313f4c55d powerpc/signal32: Fix sigset_t conversion when copying to user
On PPC64, put_sigset_t converts a sigset_t to a compat_sigset_t
before copying it to userspace. There is a typo in the case that
we have 4 words to copy, meaning that we corrupt the compat_sigset_t.

It appears that _NSIG_WORDS can't be greater than 2 at the moment
so this code is probably always optimised away anyway.

b97021f855 powerpc: Fix atomic_xxx_return barrier semantics
The Documentation/memory-barriers.txt document requires that atomic
operations that return a value act as a memory barrier both before
and after the actual atomic operation.

Our current implementation doesn't guarantee this. More specifically,
while a load following the isync can not be issued before stwcx. has
completed, that completion doesn't architecturally means that the
result of stwcx. is visible to other processors (or any previous stores
for that matter) (typically, the other processors L1 caches can still
hold the old value).

This has caused an actual crash in RCU torture testing on Power 7

This fixes it by changing those atomic ops to use new macros instead
of RELEASE/ACQUIRE barriers, called ATOMIC_ENTRY and ATMOIC_EXIT barriers,
which are then defined respectively to lwsync and sync.

I haven't had a chance to measure the performance impact (or rather
what I measured with kernel compiles is in the noise, I yet have to
find a more precise benchmark)

a9a8f77ac7 powerpc: Remove buggy 9-year-old test for binutils < 2.12.1
Recent binutils refuses to assemble AltiVec opcodes when in e500/SPE
mode, as some of those opcodes alias the "SPE" instructions.  This
triggers an ancient binutils version check even when building a kernel
with CONFIG_ALTIVEC disabled.

In theory, the check could be conditionalized on CONFIG_ALTIVEC, but in
practice it has long outlived its utility.  It is virtually impossible
to find binutils older than 2.12.1 (released 2002) in the wild anymore.
Even ancient RedHat Enterprise Linux 4 has binutils-2.14.

To fix the kernel build when done natively on e500 systems with this new
binutils, the test is simply removed.

187b9f2aa7 powerpc/book3e-64: Fix debug support for userspace
With the introduction of CONFIG_PPC_ADV_DEBUG_REGS user space debug is
broken on Book-E 64-bit parts that support delayed debug events.  When
switch_booke_debug_regs() sets DBCR0 we'll start getting debug events as
MSR_DE is also set and we aren't able to handle debug events from kernel
space.

We can remove the hack that always enables MSR_DE and loads up DBCR0 and
just utilize switch_booke_debug_regs() to get user space debug working
again.

We still need to handle critical/debug exception stacks & proper
save/restore of state for those exception levles to support debug events
from kernel space like we have on 32-bit.

4cecf6d401 sched, x86: Avoid unnecessary overflow in sched_clock
(Added the missing signed-off-by line)

In hundreds of days, the __cycles_2_ns calculation in sched_clock
has an overflow.  cyc * per_cpu(cyc2ns, cpu) exceeds 64 bits, causing
the final value to become zero.  We can solve this without losing
any precision.

We can decompose TSC into quotient and remainder of division by the
scale factor, and then use this to convert TSC into nanoseconds.

cd12909cb5 xen: map foreign pages for shared rings by updating the PTEs directly
When mapping a foreign page with xenbus_map_ring_valloc() with the
GNTTABOP_map_grant_ref hypercall, set the GNTMAP_contains_pte flag and
pass a pointer to the PTE (in init_mm).

After the page is mapped, the usual fault mechanism can be used to
update additional MMs.  This allows the vmalloc_sync_all() to be
removed from alloc_vm_area().

Fixes the PS3 bootup hang introduced in 3.0-rc1 by:

  commit 317f394160
  sched: Move the second half of ttwu() to the remote cpu

Move the PS3's LV1 EOI call lv1_end_of_interrupt_ext() from ps3_chip_eoi()
to ps3_get_irq() for IPI messages.

If lv1_send_event_locally() is called between a previous call to
lv1_send_event_locally() and the coresponding call to
lv1_end_of_interrupt_ext() the second event will not be delivered to the
target cpu.

The PS3's SMP IPIs are implemented using lv1_send_event_locally(), so if two
IPI messages of the same type are sent to the same target in a relatively
short period of time the second IPI event can become lost when
lv1_end_of_interrupt_ext() is called from ps3_chip_eoi().

d715e433b7 powerpc: Copy down exception vectors after feature fixups
kdump fails because we try to execute an HV only instruction. Feature
fixups are being applied after we copy the exception vectors down to 0
so they miss out on any updates.

We have always had this issue but it only became critical in v3.0
when we added CFAR support (breaks POWER5) and v3.1 when we added
POWERNV (breaks everyone).

6d1e2c6c1a powerpc: panic if we can't instantiate RTAS
I had to debug a strange situation where all manner of things were
failing. SMT threads, storage and network were all completely broken.

The root cause was we couldn't find enough memory to instantiate RTAS -
this was a network install so the initrd was huge.

Instead of limping along and failing in mysterious ways we should just
panic up front if RTAS exists and we can't allocate space for it.

e88d246871 sparc: Stash orig_i0 into %g6 instead of %g2
As per the comments added by this commit, %g2 turns out to not be a
usable place to save away orig_i0 for syscall restart handling.

In fact all of %g2, %g3, %g4, and %g5 are assumed to be saved across
a system call by various bits of code in glibc.

%g1 can't be used because that holds the syscall number, which would
need to be saved and restored for syscall restart handling too, and
that would only compound our problems :-)

This leaves us with %g6 and %g7 which are for "system use".  %g7 is
used as the "thread register" by glibc, but %g6 is used as a compiler
and assembler temporary scratch register.  And in no instance is %g6
used to hold a value across a system call.

Therefore %g6 is safe for storing away orig_i0, at least for now.

1d299bc773 sparc: Fix handling of orig_i0 wrt. debugging when restarting syscalls.
Although we provide a proper way for a debugger to control whether
syscall restart occurs, we run into problems because orig_i0 is not
saved and restored properly.

Luckily we can solve this problem without having to make debuggers
aware of the issue.  Across system calls, several registers are
considered volatile and can be safely clobbered.

Therefore we use the pt_regs save area of one of those registers, %g2,
as a place to save and restore orig_i0.

Debuggers transparently will do the right thing because they save and
restore this register already.

bc9f54498e PM / shmobile: Avoid restoring the INTCS state during initialization
The SH7372 PM domain initialization routine calls pd_power_up()
that executes the domain's .resume() callback, if present, and for
the A4R domain this callback attepmts to restore the INTCS state from
uninitialized data.  To avoid that, introduce __pd_power_up() that
will only execute the domain's .resume() callback if its second
argument is 'true' and make the SH7372 PM domain initialization
use it with 'false' as its second argument.  Redefine pd_power_up()
as a wrapper around __pd_power_up().

ed13ec58bf perf/x86: Enable raw event access to Intel offcore events
Now that the core offcore support is fixed up (thanks Stephane) and we
have sane generic events utilizing them, re-enable the raw access to
the feature as well.

Note that it doesn't matter if you use event 0x1b7 or 0x1bb to specify
an offcore event, either one works and neither guarantees you'll end
up on a particular offcore MSR.

57d1c0c03c perf/x86: Fix PEBS instruction unwind
Masami spotted that we always try to decode the instruction stream as
64bit instructions when running a 64bit kernel, this doesn't work for
ia32-compat proglets.

7a2512b744 [S390] incorrect note program header
'readelf -n' on the s390 vmlinux file generates lots of warnings about
corrupt notes. The reason is that the 'NOTE' program header has incorrect
file and memory sizes. The problem is that the section following the
NOTES section do not switch to a different phdr and they get added to
the NOTE program section. Add a dummy entry to the linker script that
switches to the data phdr before the start of the RODATA section.

a34dbfb03f ARM: 7160/1: setup: avoid overflowing {elf,arch}_name from proc_info_list
setup_processor copies the arch_name and elf_name fields out of the
selected proc_info_list into two fixed size buffers.

Since the proc_info_list structure is defined in a proc_*.S assembly
file, this can lead to subtle errors if the strings defined there are
too long (for example, corrupting the machine ID).

This patch uses snprintf instead of sprintf to ensure that these buffers
are not overrun.

57e6319dd6 vrtc: change its year offset from 1960 to 1972
Real world year equals the value in vrtc YEAR register plus an offset.
We used 1960 as the offset to make leap year consistent, but for a
device's first use, its YEAR register is 0 and the system year will
be parsed as 1960 which is not a valid UNIX time and will cause many
applications to fail mysteriously. So we use 1972 instead to fix this
issue.

Updated patch which adds a sanity check suggested by Mathias

This isn't a change in behaviour for systems, because 1972 is the one we
actually use. It's the old version in upstream which is out of sync with
all devices.

Fix this by adding omap1_clk_late_init() that only reprograms dpll1
if the bootloader rate is less than 60MHz. This also allows removing

The following commits break cpuidle on SH-Mobile ARM:

46bcfad cpuidle: Single/Global registration of idle states
e978aa7 cpuidle: Move dev->last_residency update to driver enter routine; remove dev->last_state

This patch remedies these issues by up-porting the SH-Mobile
code to fit with the above introduced framework changes.

It is worth noting that the new code becomes significantly cleaner,
so these framework changes are very welcome. At the same time this
breakage could probably have been avoided by grepping for "last_state"
and "cpuidle_register_driver".

750a7eee73 drivers: sh: Generalize runtime PM platform stub.
The runtime PM platform support stub in use by ARM-based SH/R-Mobile
platforms contains nothing that's specifically ARM-related and instead of
wholly generic to anything using the clock framework.

The recent runtime PM changes interact rather badly with the lazy
disabling of clocks late in the boot process through the clock framework,
leading to situations where the runtime suspend/resume paths are entered
without a clock being actively driven due to having been lazily gated
off.

In order to correct this we can trivially tie in the aforementioned stub
as a general fallback for all SH platforms that don't presently have
their own runtime PM implementations (the corner case being SH-based
SH-Mobile platforms, which have their own stub through the hwblk API --
which in turn has bitrotted and will be subsequently adapted to use the
same stub as everyone else), regardless of whether the platforms choose
to define power domains of their own or not.

This fixes up regressions for clock framework users who also build in
runtime PM support without any specific power domains of their own, which
was previously causing the serial console to be lost when warring with
lazy clock disabling.

5c20019713 MIPS: ASID conflict after CPU hotplug
I am running SMP Linux 2.6.37-rc1 on BMIPS5000 (single core dual thread)
and observe some abnormalities when doing system suspend/resume which I
narrowed down to cpu hotplugging. The suspend brings the second thread
processor down and then restarts it, after which I see memory corruption
in userspace. I started digging and found out that problem occurs because
while doing execve() the child process is getting the same ASID as the
parent, which obviously corrupts parent's address space.

Further digging showed that activate_mm() calls get_new_mmu_context() to
get a new ASID, but at this time ASID field in entryHi is 1, and
asid_cache(cpu) is 0x100 (it was just reset to ASID_FIRST_VERSION when
the secondary TP was booting).

So, get_new_mmu_context() increments the asid_cache(cpu) value to
0x101, and thus puts 0x01 into entryHi. The result - ASID field does
not get changed as it was supposed to.

My solution is very simple - do not reset asid_cache(cpu) on TP warm
restart.

6fd36ba021 x86, ioapic: Only print ioapic debug information for IRQs belonging to an ioapic chip
with "apic=verbose" the print_IO_APIC() function tries to print
IRQ to pin mappings for every active irq. It assumes chip_data
is of type irq_cfg and may cause an oops if not.

As the print_IO_APIC() is called from a late_initcall other
chained irq chips may already be registered with custom
chip_data information, causing an oops. This is the case with
intel MID SoC devices with gpio demuxers registered as irq_chips.

064a59b6dd x86/mrst: Avoid reporting wrong nmi status
Moorestown/Medfield platform does not have port 0x61 to report
NMI status, nor does it have external NMI sources. The only NMI
sources are from lapic, as results of perf counter overflow or
IPI, e.g. NMI watchdog or spin lock debug.

Reading port 0x61 on Moorestown will return 0xff which misled
NMI handlers to false critical errors such memory parity error.
The subsequent ioport access for NMI handling can also cause
undefined behavior on Moorestown.

This patch allows kernel process NMI due to watchdog or backrace
dump without unnecessary hangs.

lapic timer calibration can be combined with tsc in platform
specific calibration functions. if such calibration result is

bb84ac2d3a x86/apic: Do not clear nr_irqs_gsi if no legacy irqs
nr_legacy_irqs is set in probe_nr_irqs_gsi, we should not clear
it after that. Otherwise, the result is that MSI irqs will be
allocated from the wrong range for the systems without legacy
PIC.

056879d2f2 ARM: mach-shmobile: sh7372 A3SP no_suspend_console fix
If "no_suspend_irq" is used on the sh7372 Mackerel board
with v3.2-rc1 then Suspend-to-RAM fails because the serial
console tries to write to the SCIF driver even though the
power domain is turned off.

This patch checks the state of "console_suspend_enabled"
to see if A3SP should be forced enabled.

4b91b6fb86 arm/tegra: Don't create duplicate gpio and pinmux devices
*_pinmux_init() register the GPIO and pinmux devices so that they're ready
before any other device needs them.

*_pinmux_init() are also called by board-dt.c in order to set up the GPIO
and pinmux configurations. In this case, if we register the devices, they
end up being probed once due to this registration, and a second time due
to a device-tree node (or vice-versa). The second probe fails since the
memory regions are already requested. Besides, we don't actually want the
duplicated devices.

To avoid this duplicate registration, modify *_pinmux_init() to check
whether it's running on a DT machine. If not, register the pinmux devices.
If so, don't register them.

Finally, modify board-dt.c to call the *_pinmux_init() after all devices have
been instantiated from device-tree. This allows the GPIO and pinmux devices
to be instantiated and initialized before calling functions to configure the
hardware.

This has one disadvantage: The pinmux and GPIO initialization now happens
after /all/ devices are instantiated, rather than after just gpio and
pinmux but before anything else. So the correct HW configuration is not
in place when e.g. the SD/MMC device is probed. Long-term, this should be
solved by doing both:

a) Initializing the HW state from DT nodes during GPIO and pinmux device
   probe.
b) Using the deferred driver probe mechanism, so that drivers can defer
   their probe until after the gpio and pinmux drivers have probed.

8428e84d42 ARM: 7150/1: Allow kernel unaligned accesses on ARMv6+ processors
Recent gcc versions generate unaligned accesses by default on ARMv6 and
later processors. This patch ensures that the SCTLR.A bit is always
cleared on such processors to avoid kernel traping before
alignment_init() is called.

4f1a1eb530 MIPS: Kernel hangs occasionally during boot.
The Kernel hangs occasionally during boot after "Calibrating delay loop..".
This is caused by the c0_compare_int_usable() routine in cevt-r4k.c
returning false which causes the system to disable the timer and hang later.
The false return happens because the routine is using a series of four calls
to irq_disable_hazard() as a delay while it waits for the timer changes to
propagate to the cp0 cause register. On newer MIPS cores, like the 74K, the
series of irq_disable_hazard() calls turn into ehb instructions and can take
as little as a few clock ticks for all 4 instructions. This is not enough of
a delay, so the routine thinks the timer is not working.  This fix uses up
to a max number of cycle counter ticks for the delay and uses
back_to_back_c0_hazard() instead of irq_disable_hazard() to handle the
hazard condition between cp0 writes and cp0 reads.

66f5ddf30a x86/mce: Make mce_chrdev_ops 'static const'
Arjan would like to make struct file_operations const, but
mce-inject directly writes to the mce_chrdev_ops to install its
write handler. In an ideal world mce-inject would have its own
character device, but we have a sizable legacy of test scripts
that hardwire "/dev/mcelog", so it would be painful to switch to
a separate device now. Instead, this patch switches to a stub
function in the mce code, with a registration helper that
mce-inject can call when it is loaded.

Note that this would also allow for a sane process to allow
mce-inject to be unloaded again (with an unregister function,
and appropriate module_{get,put}() calls), but that is left for
potential future patches.

e94e05eae9 arm/mxs: fix mmc device adding for mach-mx28evk
The merge commit "526b264 Merge branch 'imx/cleanup' into imx/devel"
left a duplicated mx28_add_mxs_mmc() call, which causes the problem
below during boot.

  kobject_add_internal failed for mxs-mmc.1 with -EEXIST, don't try
  to register things with the same name in the same directory.

The patch removes this leftover and also change mmc0 adding to align
with mmc1.
