This patch adds definition of the Samsung S5C73M3 camera specific
image format. V4L2_PIX_FMT_S5C_UYVY_JPG is a two-planar format,
the first plane contains interleaved UYVY and JPEG data followed
by meta-data. The second plane contains additional meta-data needed
for extracting JPEG and UYVY data stream from the first plane.

Fix the documentation of the Y4 and Y6 formats.
Fix a poll() issue, add support for enum_frmsizes, set the proper
parent device and fix a few compliance issues.
Tested with an actual Connectix B&W parallel port webcam, both on a
little-endian and a big-endian platform. This driver has never been
so good, doing 320x240 at 1 frame per second :-)
I know, nobody cares, but still it is cool that linux can still support
this old webcam.

This patch addresses several issues pointed by Randy Dunlap
<rdunlap@xenotime.net> at changeset ece722c:
- In the generated index.html file, "media" is listed first, but it
should be listed in alphabetical order, not first.
- The generated files are (hidden) in .tmpmedia/
- The link from the top-level index.html file to "media" is to
media/index.html, but the file is actually in .tmpmedia/media/index.html
- Please build docs with and without using "O=builddir" and test that.
- Would it be possible for media to have its own Makefile instead of
merging into this one?
Due to the way cleandocs target works, I had to rename the media DocBook
to media_api, otherwise cleandocs would remove the /media directory.
